What Services Does MAP Offer to Patients and Physicians?

Reimbursement Counseling
MAP counselors will see if MACUGEN is covered by your patient's insurance.

If MACUGEN is not covered, other sources of coverage will be investigated

These may include federal, state, and local assistance programs

MAP counselors can also assist you and your patients with the prior-authorization process, claim denials, and the appeals process

Patient Assistance Program (PAP)
The Patient Assistance Program is available to eligible patients if coverage for MACUGEN is not identified through insurance or any other programs, or if coverage is denied.

Your patient will be considered for PAP if:

He or she does not have insurance coverage for MACUGEN
Coverage cannot be found through insurance or other programs
Coverage for MACUGEN is denied and appeals to your patient’s insurer fail
The patient meets the PAP criteria

Reimbursement Counseling Services

MAP counselors will provide you with personalized assistance to help facilitate reimbursement for and patient access to MACUGEN. MAP counselors provide a range of services and will be available to help you with the following:

Benefit verification for MACUGEN
Prior-authorization assistance, when required by the insurance provider
Claims-tracking assistance, when physicians submit claims for MACUGEN
Claims-filing assistance, when filing claims for MACUGEN
Assistance with appeals, when MACUGEN has been denied by an insurer
Alternate-coverage research, for those patients without coverage for MACUGEN

Getting Started With MAP

To apply to MAP, you and your patient will be required to fill out the MAP Enrollment Form. The form lists what you will need to provide to apply for the program.
